My personal and professional experience, knowledge and training ensures I offer the right support at the right time. I am committed to ensure that our elders, including those experiencing dementia; receive kind, respectful and person centred care no matter where they live. I aim to ensure that wherever possible people experience an informed and peaceful end of life transition and to support them if their wish is to die at home, or not die alone. I offer genuine care and kindness; and appropriate and timely information, options, resources and support. I am intuitive, compassionate, and professional. All services are delivered in a respectful and supportive manner and in a loving, peaceful environment.
The service packages include:

AT HOME OR RESIDENTIAL CARE FACILITY SUPPORT
These packages are available to clients, family and significant others who are either in their own home or a residential care facility. Where possible and appropriate services can be offered via zoom/phone

One hour face to face, phone or Zoom consultation to determine if the services will meet your needs
Covering all aspects from planning to end of life
RATE: Purchase as individual packages as required
The services offered include EARLY ONSET or a RECENT DIAGNOSIS. In home /facility and community engagement support; encourage the maintenance of independence; and continue or introduce participation in activities of interest.
RATE: All packages are customised and uniquely developed based on the needs of the individual and family/significant others.
Pet therapy is available and I am supportive of pets in care.
Early Care Planning
I support you to ensure your end-of-life wishes, preferences and options, before and after your death, are clear
RATE: Once off – x2 sessions for $320
End Of Life
Provide support to ensure a person’s end of life wishes are honoured. This includes immediate response, end of life transition, vigiling, and after death support

RATE: Half day – $320/evening – $240/full day $580/overnight (active $580 or passive $200)/after death $160. 2 or 4 hourly packages also available
Travel & Accommodation
All services can be offered in the comfort of your home or the facility you reside in. Travel that occurs outside a 10km radius from my home will incur an additional charge. Overnight stays will incur accommodation costs.
RESIDENTIAL CARE FACILITY SUPPORT
Supporting the transition to facility care using a person-centred approach and with ease and simplicity. Other residential care packages can be offered after the initial period.
RATE: From $320
Support to a resident living in an aged care facility. This may be occasional, regular, immediate, or for compassionate reasons.
RATE: 2 or 4 hourly packages available from $160
FAMILY AND SIGNIFICANT OTHER SUPPORT
RATE: $50/hour for weekday day rate, $80/hour weekend, $200 passive overnight, active overnight by mutual agreement
